I give my consent for emergency medical treatment for my child, should the necessity arise.

Covid-19 Declaration

1. I will check the temperature of my child before attending the camp each morning.
2. I will not attend camp if within 14 days prior to the start of camp, I or a member of my household has been in contact with someone with Covid-19
3. I have explained to the best of my ability and understanding the current restrictions to my child in regards to social distancing and hand hygiene.
4. I will not attend camp if within 14 days prior to the start of camp, I or a member of my household has experienced symptoms Covid-19

Terms & Conditions

1. Due to current restrictions, some activities will not be able to go ahead as normal.
2. Payment is regarded as acceptance of the terms and conditions.
3. Full payment is needed to secure the place on the camp.
4. No refunds are made for cancellations unless medical cert is supplied.
5. Any participant whose conduct is deemed by the camp manager to be detrimental to the best interest of the programme will be dismissed-no refund given.
6. Spitting and biting under any circumstance will not be tolerated. Parents will be phoned and the child must be removed from the camp.
7. While every reasonable care will be taken for the care and well being of your child, Shoreline Sports Park cannot be held responsible for any injuries incurred or loss/damage to property.
8. Shoreline Sports Park shall not be responsible for children left unsupervised at the Park before 10.00am
